ABSTRACT

OBJECTIVE:To provide referenc e for the construction and software development of knowledge base for rational use of TCM decoction pieces. METHODS :By reviewing the literatures on rational drug use software and TCM decoction pieces in recent years ,the clinical characteristics of rational drug use of TCM decoction pieces as well as the characteristics and shortcomings of existing rational drug use software in the detection of rational drug use of TCM decoction pieces were analyzed , and the core contents and difficulties in the construction of knowledge base of rational drug use software of TCM decoction pieces were summarized. RESULTS & CONCLUSIONS :Clinical application of TCM decoction pieces was mainly based on “syndrome differentiation”,which reflected the unity of dialectics ,treatment,prescription selection and medication. Therefore ,the consideration of the rationality of clinical use of TCM decoction pieces could not blindly imitate the evaluation method of chemical medicine. Current rational drug use software was not based on the theoretical system of traditional Chinese medicine ,and it was not comprehensive and mature in the aspect of rational drug use review of TCM decoction pieces ,and lacks the knowledge base that could meet the requirements of rational use of TCM decoction pieces. Therefore ,it is necessary to construct a set of knowledge base which can meet the evaluation requirements of “consistency of principle ,method and prescription use ”of TCM decoction pieces under the guidance of TCM theoretical system. Its contents include that patient information collection ,construction of knowledge base related to diseases and syndromes ,selection of processed products of TCM dec oction pieces ,addition andsubtraction of clinical symptoms ,selection taboo of varieties of TCM decoction pieces , compatibility taboo , combined application of Chinese patent medicine or chemical medicine , dosage of TCM decoction pieces , total dosage and tastquantity of each prescription , special de coction drugs , medication methods and administration frequency ,etc. There are still some difficulties in the development of rational drug use software of TCM decoction pieces ,such as the construction of disease and syndrome related knowledge base and the difficulty in judging the rationality of clinical symptom addition and subtraction.

ABSTRACT

OBJECTIVE:To explore the role of pharmacists i n developi ng the outpatient and emergency prescription pre-audit with the help of rational drug use software. METHODS :With the help of rational drug use software ,outpatient and emergency prescription pre-aduit was conducted during Jan.-Apr. 2020;the intervention effects of irrational prescriptions were analyzed. The prescriptions from Sept. to Dec. in 2019 were included in the control group (only for post review ),and the prescriptions from Jan. to Apr. in 2020 were included in the intervention group (pre-audit and post review );the unreasonable prescriptions were compared before and after the intervention. The prescriptions intercepted by the software system from Nov. to Dec. in 2019 were further selected as the interception control group ,and the prescriptions intercepted from Jan. to Feb. in 2020 were selected as the interception intervention group ;the interception situation of prescriptions before and after the intervention was compared ,and the influence of pre-audit on physicians ’prescription behavior was evaluated. In addition ,108 992 prescriptions in Apr. 2020 were selected for post review using the traditional model (i.e. complete manual audit )and the information model (i.e. software-assisted manual audit )respectively;the effects of two models were compared. RESULTS :From Jan. to Apr. in 2020,a total of 2 393 prescriptions were pre-audited by pharmacists ,among which 1 387 prescriptions were reasonable (57.96%)and 1 006 prescriptions were unreasonable and intervened (42.04%). After pharmacist intervention ,983 prescriptions were modified by doctors ,and the success rate of intervention was 97.71% . Compared with control group ,unreasonable rate of outpatient and emergency prescriptions,that of each type of prescriptions were all decreased significantly (P<0.001). Compared with interception control group(1 402 pieces),the number of intercepted prescriptions (721 pieces)was significantly reduced in interception intervention group (P<0.001). Compared with traditional model , the number of prescriptions requiring manual post review and E-mail:xss306@126.com “false positive ” prescriptions were decreased significantly # under information model , and the number of discovered unreasonable prescriptions were increased significantly (P< 0.001). CONCLUSIONS :With the help of rational drug use software ,prescription pre-audit by pharmacists can significantly improve reasonable rate of outpatient and emergency prescriptions. The work intensity of pharmacists in post review of prescriptions can be reduced and the accuracy of prescription review can be improved in the information mode.

ABSTRACT

OBJECTIVE:To discuss the application of rational drug use software system in drug dispensing in outpatient depart-ment of our hospital. METHODS:The application of rational drug use software system (included clinical decision support soft-ware,drug dispensing software and drug management software) in prescribing (warning in advance),dispensing (intervention in the event)and the prescription review(the post review)in outpatient department of our hospital were all introduced. Outpatient pre-scription checking and intervention were collected from our hospital after the application of rational drug use software system to evaluate the effect of the software system. RESULTS & CONCLUSIONS:Rational drug use software system is adopted to realize scientific,convenient and express monitoring and management of prescription drug use in advance,in the course and afterwards. A total of 721 507 outpatient prescriptions were checked in our hospital from Jan. to May in 2015;0.17‰prescriptions were intercept-ed by system warning;system pointed out and pharmacists had checked 23.25% prescriptions;the rate of qualified prescription was more than 99.96%. After pharmacists intervention,various types of irrational prescriptions decreased significantly (P<0.01). It is suggested that pharmacists should make full use of information system,at the same time,optimize and improve the system through active exploration so as to improve rational drug use.
